BJC Home Care Services offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high-tech respiratory care. Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs as well as the Lifeline emergency response system. BJC Home Care Services provides care to thousands of patients in both Missouri and Illinois. Hospice, the final stage of BJC's continuum of care, is a special kind of caring for patients with a life-limiting illness. Services are provided in the comfort of the patient's home or skilled nursing facility. The Hospice staff are sensitive to the physical, psychosocial, emotional and spiritual needs of terminally ill adult and pediatric patients and their families. We provide a multi-disciplinary team of healthcare professionals and volunteers, specially trained in symptom management, pain control, counseling and bereavement services for the dying. Our Hospice services include alternative therapies such as music, art and massage therapy. Our Hospice programs provide palliative care by helping patients manage their pain and symptoms while living their lives with daily peace and dignity. Join Us! Role Purpose Under general supervision and according to established policies and procedures, assists in the design, testing, training, implementation and support of clinical systems for assigned sites(s), ensuring that clinical processes are accurately reflected and supported. Functions as the informatics coordinator of value realization programs and other regulatory and quality initiatives. Performs analysis of current processes and new/existing technologies in order to assist in the management and processing of data information and knowledge to support clinical practice and the delivery of patient care. This position applies knowledge of nursing and informatics to assist in the design, development, and ongoing modification of healthcare systems Responsibilities Education - Acts as the “Subject Matter Expert” responsible for providing insight into department operational and clinical processes and uses clinical process knowledge to integrate application use into those clinical processes. - Educates all levels of staff throughout the agency, including clinical orientation and competency assessment. - Provide professional development to nursing staff, other clinical staff as well as students, interns, residents for clinical rotation programs - Performs annual educational needs assessment; evaluates education programs, projects and plans future organizational education needs. - Assists in the development and revisions of on-line annual education. - Responsible for the new employee clinical orientation program, and initial/annual clinical competencies for nursing staff within a clinical skills lab environment. Informatics - Provides software updates to staff at team meetings. - Provides instruction for all new PC or laptop users in home health and hospice. - Tests and troubleshoots new/updated software. - Develops, plans, and provides training for all staff levels in various aspects of software - Demonstrates expertise in standards and best practices in the evaluation of technical solutions. Collaboration - Collaborates with clinicians and organizational leaders to identify data/information reporting needs, collects and analyzes data and identifies trends to improve organizational performance and clinical outcomes. - Collaborates with staff and representatives of other organizations to develop, present and/or evaluate clinically focused training, continuing education and/or professional development opportunities. Performance Improvement - Assists in problem solving, promotes the implementation of the health care system applications and quality improvement assessments and projects.
